  • Mi Via Participant Guidebook: (Removed for revision 3/31/11)
    The Mi Via guidebook describes the Self-Directed Waiver Program in detail. It will assist individuals through the process of applying for and using the program. The guidebook includes many of the Mi Via forms with instructions, and provides a complete glossary of terms.
