Mi Via is under constant program changes and refinements we hope you will find the information below useful. 

Updated Forms and Toolkits Have Been Mailed to all Participants - 7/29/2011

We recently mailed out the following updated forms: Employee 2-Week Timesheet and Toolkit, and the Vendor Non-Timesheet and Invoice Toolkit. Please begin using these updated forms immediately. The updated forms contain the new service codes that were added due to the new Mi Via Waiver and Regulations. Although you can start using the updated forms right away, if you haven’t started a new Plan yet with the new service codes, you will still need to use the old service codes until your old Plan ends. The important thing to remember is that when you submit timesheets and Payment Request Forms (PRFs), you use the codes as they are listed on your current Plan. If you have questions about which codes to use, please refer to your SSP, contact your Consultant, or call the ACS Help Desk at 1-866-916-0310.
